cover.xhtml

An XHTML file for the cover (sometimes called cover.xhtml) is not to be added and if found should be removed (This rule may not apply to fixed layout). cover.xhtml is a default feature of an exported InDesign ePub.

If a cover.xhtml does exist it will also need to be removed from the content.opf file.
